Generally speaking, a tooth can begin to hurt for various reasons: due to caries, pulpitis, periodontitis, periostitis, etc. Often, even gum disease provokes toothache. The most striking example is the difficulty of teething when the gum becomes inflamed and swells so badly that the pain radiates to neighboring teeth, and sometimes even to the ear or throat.

With difficult teething wisdom teeth, often the best way to get rid of pain and possible complications is to remove it.

Dental diseases of carious and non-carious origin also determine the different nature of the pain. For example, the most severe pains most often occur due to structural changes in the soft tissues inside the tooth - the pulp.

Pulp is a neurovascular bundle that consists of a huge number of nerve cells. With inflammation and destruction of this tissue, acute and severe pains can occur that literally a person cannot find a place for himself.

Unfortunately, not everyone makes the right decision about what they should do when a toothache occurs: someone waits until the last and suffers, someone swallows pills in the hope that everything will "resolve" by itself. There are many cases when people wait unreasonably for a teething wisdom tooth, although such a wait against a background of severe inflammation can be very dangerous from the point of view of developing serious complications.

What should I do if my head starts to hurt badly from toothache?

Indeed, toothache can sometimes give to the head, thereby provoking severe migraine. Typically, such pains are of short-term "shooting" nature. However, with “crooked” growing wisdom teeth, which due to improper position in the gum damage the roots of neighboring teeth during their growth, the headache can be constant, becoming chronic. In such cases, the wisdom tooth provoking it should be eliminated as soon as possible, after which the headache quickly passes.

Sometimes a headache can be associated with the presence of deep caries in a tooth. As soon as the tooth is properly treated, the headache will pass.

Possible Causes of Toothache

Suppose you have a toothache and you don’t know what to do: either suffer a couple of days, in the hope that the pain itself will go away, or it’s better to immediately make an appointment with the dentist ... Therefore, let's talk about the possible root causes of toothache and mechanisms of its appearance, understanding of which will help to make the right decision in different situations.

So, let's start with pain, which are characteristic signs of caries. Already with the initial forms of caries, a feeling of soreness and increased sensitivity of foci of demineralization of enamel (that is, areas of enamel with partially washed mineral components) may appear. At this stage, to prevent further destruction of hard tissues, a course of remineralizing therapy should be taken as soon as possible (at least, use remineralizing toothpastes).

However, severe pain can appear only when the carious destruction reaches the enamel-dentin border - in this case, tooth tissues are sensitive to various irritants (cold, sweet, etc.). In such situations, to quickly get rid of unpleasant sensations, it is enough to simply eliminate the irritant, and the pain will immediately pass. However, of course, for a full treatment without a seal there will no longer be enough.

Pain in medium and deep caries occurs due to the fact that the dentin layer is penetrated by many dentinal tubules that feed the tooth and determine its sensitivity (and also have a certain connection with the dental “nerve”). When the dentin layer is destroyed, something like a dentin “wound” appears, which responds to the action of stimuli.

With deep caries, a tooth can become very sensitive to a variety of irritants.

Question to the dentist: “Why does the tooth hurt under the old filling, and what should be done in this situation?”

If the seal was delivered more than 5-7 years ago, then significant shrinkage (pressing) of the seal is possible. At the same time, its adherence to the edges of the tooth is disturbed, therefore, with a "live" pulp, it sometimes painfully feels an irritant (cold, hot).

A tooth that has long been sealed in the canals with a high-quality sealed filling usually does not bother if no errors occurred during treatment. If the old seal ceases to meet the requirements of tightness, or even flies out in pieces, then a dead tooth begins to hurt due to infection in the already sealed canals. In addition, mistakes could be made during treatment: an incompletely sealed canal with an infection left in it, a broken dental instrument in the canal, etc. All this can also cause inflammation at the root of the tooth and pain over time.

In any case, if there is pain in the tooth under an old filling, you should immediately consult a dentist, as this is a sure sign of the development of pathological processes.

Now let's see what to do with tooth pain with pulpitis.

In acute pulpitis, the neurovascular bundle inside the tooth undergoes catastrophic changes. Inflammation due to the action of microbes and their toxins on the living tissue of the “nerve” causes its edema and gradual necrosis.

It is not difficult to imagine: the pulp gradually begins to “swell”, increasing in size, and when the pulp chamber is not opened, it strongly presses against the inner walls. The nerve endings are compressed, and acute pain occurs, which can only be relieved by strong painkillers.

Thus, in most cases with pulpitis, canal treatment will be required, that is, a simple filling will not be enough here.

With pulpitis, an infection of the neurovascular bundle and its inflammation occur, which is accompanied by very severe pain.

This is interesting: why sometimes after installing arsenic in a tooth there is a sharp pain, and what should be done to prevent this from happening?

Doctors of the old school still continue to put arsenic paste, and legends and myths about a terrible dental monster named Arsenic appear among the people. If you correctly complete all the stages of the paste and withstand the necessary time spent in the tooth, then there will be no direct toxic (negative) effect leading to severe toothache.

However, in practice it is difficult to control the process of finding the substance in the tooth by the clock, especially since the dosage of the paste is also very arbitrary (ball head No. 1). Many dentists still wonder, even if they saw or know what size this head is, how realistic it is to determine its volume for each individual patient and his tooth.

A study by scientists on the effects of arsenic acid on the tissue around the root revealed the risks of serious inflammatory processes, including cysts, which often leads to tooth extraction. It is for these many reasons that it is easier to abandon arsenic, even in budget dentistry.

Folk remedies against toothache, and in which cases they can be used

What to do if a toothache is taken by surprise - for example, during a business trip, or during long holidays? If it is not possible to see a dentist in the near future, you can temporarily help yourself.

Traditional methods of getting rid of toothache have been known for a long time. Unfortunately, not all of them are equally effective, but in some cases they can provide some effect.

One of the popular remedies is rinsing the mouth with a warm broth of sage. One tablespoon of sage should be poured in a glass of boiling water, and insisted for an hour. Rinse with a solution every 10-15 minutes.

A decoction based on sage leaves is an old folk remedy for relieving various types of toothache.

The anti-inflammatory effect of sage has been known for a long time. It can also be used in cases where the gum is swollen, a flux has appeared, and also when a wisdom tooth erupts and pain gives off along the branches of a nerve in the ear, temple or nape. During rinsing, it is important to use a warm, almost hot solution and keep it near a diseased tooth or gum to enhance the therapeutic effect.

Rinses can be carried out with tea, if only the warming went from the inside. It is impossible to warm a tooth outside! There were times when the night before a meeting with the dentist, people heated a bad tooth, using a heating battery or a bottle of hot water as a heating pad. There are among recipes for hellish toothache and this: you need to put a piece of propolis on the tooth or in the cavity, close your mouth and hold for 15-20 minutes.   • Attach garlic to your wrist. In appropriate literature, it is recommended to firmly fix the paste from fresh garlic to the wrist, which is opposite to the diseased tooth, with a plaster. To prevent a burn, put a bandage or dense cloth under the garlic. In practice, the method is ineffective.
  • Warm up your feet in hot water. This procedure is not only a maneuver distracting from pain. With increased blood circulation in the body during the procedure of steaming the legs, the metabolism improves and toxins are removed faster from the tooth pulp: it is necessary to lower the legs in hot water and keep them for at least 15 minutes. Then you should put on warm socks and go to bed. In some cases, this method really gives effect.

The simplest and most effective way to get rid of toothache at home is, perhaps, warm rinses with a solution of soda and salt. It is only important to rinse with an almost hot solution and very often, about once every 10-15 minutes, repeating the procedure for 1-2 hours.

Frequent mouth rinses with a warm solution of salt and soda really help relieve toothache (but not in all cases.).

Such funds include, for example, Kamistad, Holisal, Kalgel, Solokoseril, etc.These gels sometimes do not last long (for 30-40 minutes), but this is enough to gain time and seek professional help from a dentist.

What to do if a wisdom tooth hurts and the gum swells near it?

With a difficult teething wisdom tooth, severe pain occurs (under the so-called gingival hood), which can spread to the jaw, cheek, throat, ear. Home rinsing can only soothe this pain for a while, but it does not fix the problem, so you need to go to the clinic - the maxillofacial surgeon or dental surgeon usually comes to the rescue.

Modern studies have shown that excision of the gingival hood brings only temporary well-being, and can lead to serious complications (phlegmon, sepsis), up to death. The only way to prevent any risks and save a person from pain is to remove the problematic wisdom tooth.

Why can a dead tooth hurt?

A tooth is called dead if there is no pulp in it (it died or was removed). It would seem, why then get sick - after all, the dental "nerve" is no longer able to fulfill its function ...

With periodontitis, the "nerve" really dies: however, then it begins to decompose right here. Pus rushes to the root of the tooth, the surrounding tissue root becomes inflamed and swollen, which causes terrible toothache.

There is another situation. After treatment of the canals, the tooth is considered “dead” and should not hurt normally. In some cases, a slight toothache after filling the canals, which is called post-sealing. Usually it lasts no more than 1-2 weeks and has a mandatory tendency to decrease every day.

Otherwise, severe pain that does not pass after channel treatment, gum swelling, redness, fever and general well-being indicate a serious problem - in this situation, you need to see a doctor again.

Complications during and after treatment of the canals are most often provoked by the dentist due to poor training, lack of experience, or equipment and materials in the clinic. Most often, pain after filling the canals occurs for the following reasons:

  • with excessive removal of filling material beyond the root;
  • when skipping a channel or channels in a tooth;
  • with poor channel processing;
  • due to violations during the passage and expansion of intra-channel instruments;
  • due to poor-quality filling;
  • due to allergies to filling material;

In what cases may you have to part with a tooth?

As mentioned above, difficult wisdom tooth eruption (pericoronitis) is the first indication for tooth extraction according to the modern approach to dental health. Of course, you can always persuade the doctor to make you only a section of the gingival hood, but where is the guarantee that the tooth "does not climb crookedly", its roots do not intertwine with the roots of neighboring teeth, that the hood does not grow again, and the inflammation does not continue again.

With pulpitis, teeth should always be preserved, but periodontitis is a special topic. Acute forms of periodontitis are easier to treat, which cannot be said about the possibility of preserving teeth with cysts. However, the cyst of the cyst is also different, therefore, in each case, the prognosis of the disease, the prospect of treatment and the importance of the tooth in the dentition and bite as a whole are determined.

Periostitis, osteomyelitis, abscesses, phlegmon and other odontogenic (from the tooth) pronounced purulent processes often determine the indications for removal.
