Let's talk about alveolitis - an unpleasant complication that often occurs after tooth extraction and is manifested by inflammation and suppuration of the hole, which reduces the rate of its healing.

Sometimes after tooth extraction, after about 1-3 days, inflammation and suppuration of the hole (alveolitis) begins - a rather dangerous complication, which is accompanied by a number of unpleasant symptoms. Alveolitis can occur after the extraction of any tooth, disrupting the normal healing process of the hole and often creating a serious threat to human health in general.

If after removing the tooth in time not to treat the hole affected by the alveolitis, then there are risks of serious complications. For example, with the further development of a purulent-necrotic process, limited osteomyelitis may form in the wound, which creates the prerequisites for the development of life-threatening abscesses and phlegmon of the jaw. How does the hole look normal after tooth extraction

After tooth extraction, bleeding from the hole usually occurs immediately (but sometimes adrenaline from the anesthetic narrows the vessels so much that the hole is almost “dry” at first and the bleeding develops only later when the patient is already at home). Normally, blood stops within 3-30 minutes, forming a blood clot in the hole, which protects the wound from adverse external influences.

The blood clot that formed after tooth extraction changes in a couple of days from burgundy to lighter with a yellowish tinge. The color in this case is determined by natural physiological processes: the red component from red blood cells (hemoglobin) is gradually washed out by the fluid from the oral cavity, but the so-called fibrin skeleton is preserved, which serves as the structural basis of the thrombus, which prevents bleeding from the hole of the extracted tooth.

If tissue healing goes smoothly, then a normal well should look something like 1-2 in 1-2 days:

Despite the characteristic color, you should not confuse a normal blood clot with pus and fear that a change in color from burgundy to lighter is already a sign of alveolitis. Moreover, you do not need to climb into the wound, as this can seriously damage the normal healing of the hole.

The process of normal healing (overgrowing) of the hole after tooth extraction should be painless.The wound heals on the principle of secondary tension, in other words - there is a gradual approximation of its edges. The main healing factor is the proliferation of granulation tissue, which is soon replaced by young bone tissue.

If the process is not complicated by alveolitis, then usually about 7 days after tooth extraction, the granulation tissue almost completely replaces the blood clot, and a young bone is actively formed. How long the hole will overgrow after tooth extraction, so that a smooth gum forms completely, depends on the nature of the intervention. The coarser the removal and the more damaged surrounding tissues, the longer the wound will heal.

According to statistics, normal healing should end in 2-3 weeks, but this does not mean that nothing happens in the hole. The complete formation of bone tissue under the gum usually ends only after 2-3 months, and then from the former hole, in which the patient was previously, there is no trace left.

The main causes of inflammation of the hole and gums

Let's imagine a bad tooth, on the roots of which there are foci of inflammation. Such foci are usually isolated by the body into the so-called granulomas (cysts) - in simple terms, these are sacs filled with infection and pus.

When the tooth is removed, the bacteria partly remain in the hole - in this case, they speak of the primary infection of the wound. Even if the dental surgeon has extracted all the roots of the tooth with or without granulomas (cysts), the infection that caused inflammation in the tissues surrounding the root cannot be completely eliminated, although it can and must be controlled. Whether it provokes alveolitis, depends, inter alia, on human immunity.

There are also risks of secondary infection of the wound, that is, when the infection enters from the outside, which can also lead to alveolitis.

For example, alveolitis after tooth extraction can develop in the following cases:

  • In the absence of a normal blood clot in the well. When the effect of a dry hole occurs immediately or after tooth extraction, the result is an empty, not filled with a protective blood clot wound, open to any infections from the oral cavity.If a protective blood clot does not form in the tooth well, the likelihood of developing alveolitis increases sharply.
  • When pushing during the tooth extraction procedure deep into the hole of infected objects: tartar, carious tooth walls, bone fragments, etc.
  • With poor care of the hole by the patient himself. Symptoms of Alveolitis

So, after a tooth is pulled out, the hole, especially if it is dry (without a blood clot), can already fester 2 days after the procedure. In this case, at first there is no acute pain, but only a slight aching pain appears, intensifying with food.

In this case, the remains of food and saliva are often determined in the hole. The edges of the gums at the site of the extracted tooth are red, when you touch them there is pain. The general condition of the person is satisfactory, there is no temperature.

At the initial stage of alveolitis, there is no acute pain, but a slight aching pain already appears.

With the further spread of purulent infection, more characteristic symptoms of alveolitis develop:

  • severe constant pain at the site of the extracted tooth;
  • the spread (irradiation) of pain to neighboring areas - from the maxillofacial zone to the head area;
  • an increase in body temperature to relatively high values ​​(up to 38) and the appearance of chills associated with it, a sensation of aches in the body and a deterioration in overall well-being;
  • the appearance of putrid halitosis as a result of suppuration of the hole;
  • the absence of a blood clot in the wound (dry hole);
  • the presence of a dirty gray coating on the surface of the healing gum (sometimes a black hole is observed);
  • enlarged submandibular lymph nodes;
  • one of the important and characteristic signs of suppuration of the socket in place of the extracted tooth is the appearance of purulent exudate from the wound, especially when pressing the gum.

With severe inflammation of the wound, temperature can rise and a feeling of chills and general weakness may appear.

What can be done at home for emergency treatment of a festering hole

If, after tooth extraction, the hole begins to fester or the other symptoms of the alveolitis described above appear, and it is not possible to get to the doctor for a consultation in the near future, then you can try to ease the condition yourself at home.

If it’s difficult to get to the doctor, then the symptoms of alveolitis can well be alleviated at home, especially if you take treatment on time.

However, it is important to keep in mind that far from all seemingly effective methods are harmless. Some lotions and rinses only exacerbate the situation, creating the prerequisites for an even greater progression of inflammation.

So, for example, most caring friends can advise you on a soda solution and 3% hydrogen peroxide for rinsing - it would seem, time-tested folk remedies for relieving inflammation. But most doctors are clearly against such a mockery of the body, because soda and peroxide, despite their antiseptic effect, can also wash out a protective blood clot, leaving the empty black hole further defenseless before infection.

With alveolitis, it is not recommended to rinse your mouth with hydrogen peroxide, as well as soda.

Relatively safe and effective remedies for the treatment of alveolitis can be natural antiseptics - for example, decoctions of chamomile and sage, which are quite suitable for use at home. But here there are important nuances:

First of all, do not deceive yourself much and relax, hoping that the hole will no longer fester. Often in such cases, the process only translates from acute to chronic. Professional treatment of alveolitis: what can you expect in the dentist's office

I have observed this problem more than once (especially in elderly patients). The gum really grows randomly, filling the void in the hole, as God puts on the soul. In the end, after about a couple of weeks the wound is formed more or less beautifully: the edges become smooth and, accordingly, the gum itself looks even.It can take its final form (especially after complex and traumatic removals) not earlier than after 3-4 months (up to six months).
