But about twenty years ago, treating caries was really painful. Today, even its most neglected form is treated in such a way that the patient does not feel even half the pain that he could have felt in Soviet times. Types of caries and sensations in their treatment

Caries at different stages of its development requires different treatments, and the patient's sensations with them also vary slightly.

Stage of development of caries: from a spot on the tooth to the defeat of the pulp chamber.

In case of caries at the spot stage, the enamel lesion site is treated first with special grinding materials, and then enamel reducing preparations are applied to it. These procedures are painless, and feel similar to thoroughly brushing your teeth with a toothbrush.

At the initial stage of a carious lesion of enamel, its almost complete restoration without the use of a drill is possible.

Sometimes, before treating tooth decay, a doctor has to remove plaque. At the same time, painful sensations practically do not occur.

With caries in the spot stage, anesthesia (injection) is not done, which most people are unjustifiably afraid of. However, it is always anesthesia that completely eliminates the appearance of even minimal sensitivity.

With superficial caries, two treatment options are possible:

  • if the lesion stain is located on the smooth wall of the tooth, it is simply ground and a restorative therapy is carried out according to the example of procedures with caries at the stain stage.
  • If superficial caries developed in fissures, the doctor usually decides to open them according to the protocol and fill them. If the area of ​​destruction did not have time to reach the dentin, this procedure will not cause pain. If there is even a hint of pain or immediately at the request of the patient, the doctor will immediately anesthetize the tooth.

Anesthetics and discomfort

For local anesthesia in dentistry, a number of drugs are used, from which the doctor chooses the one that will provide anesthesia for the required period of time.

Today, there are many drugs for local anesthesia, which can be used both in the treatment of adults and children.

Typically, the following types of painkillers are used in clinics:

  • Novocaine, Mepivacaine - short-acting anesthetics (about half an hour);
  • Lidocaine, Artikain - anesthetics of average duration (from an hour to an hour and a half);
  • Ethidocaine, Bupivacaine - long-acting drugs (over one and a half hours).

Novocaine in dentistry is slowly but surely leaving in the distant past. In most megacities and cities, they stopped using novocaine as anesthesia due to its inefficiency and increased risk of an allergic reaction to it. It was invented more than 100 years ago and has become obsolete as an anesthetic, although its importance in modern medicine cannot be underestimated (novocaine blockade, relieving vasospasm, dilution of drugs, etc.).

Novocaine and its successor, Lidocaine, which replaced the “old” anesthetic, are diluted with a 0.1% solution of adrenaline to enhance the duration and depth of action, but, unfortunately, often by eye. Most dentists deliberately turned away from such a vicious practice, choosing imported anesthetics, already diluted with adrenaline (epinephrine). This is due to past complications from an overdose of adrenaline. If you want to add an average of 1 drop per 5-10 ml of solution, then overdosing adrenaline is very easy.

Complications arise immediately in the form of fear, shortness of breath, tremors, cardiac arrhythmias, including loss of consciousness and cerebral hemorrhage.Why risk your own authority, the reputation of the institution, and most importantly, the life of the patient, when there are imported anesthetics that are completely ready for use, have quality certificates that practically do not cause complications and allergic reactions, are effective for various anesthesia techniques in dentistry ...

Novocaine (procaine): chemical formula

Anesthetics are injected with a syringe (usually karpulnogo), and depending on the group of the tooth and its location, they are pricked in different areas:

  • into the transitional fold closer to the projection of the tooth root (infiltration) to turn off the peripheral “nerves” suitable for the tooth (from the outside and inside).
  • In the area of ​​the exit of the main "nerve" for a complete "shutdown" of pain impulses to several teeth (conduction anesthesia). For example, in the treatment of lower molars, torus or mandibular anesthesia is often done, which allows you to treat 3-5 teeth immediately without pain while it is acting.
  • Intraligamentous (intracircular) anesthesia is done into the periodontal ligament with a syringe specially developed for this technique. At the same time, high efficiency of anesthesia is achieved.

It is important to understand that different patients have different thresholds for pain sensitivity and susceptibility to anesthesia: some normally tolerate cleaning the carious cavity with moderate caries without anesthesia, others are painful to treat tooth decay even when polishing damaged enamel. Therefore, the doctor himself during work makes a decision about which drug and in which place to prick.

Generally speaking, today carious cavities are treated with the most gentle methods. At the tips of modern drills there are channels for irrigation of the place of work of boron with cold water, and the burs themselves are made from unique alloys with diamond dusting, which allows literally jewelry operations inside the tooth. In addition, today in individual clinics there are laser drills that can completely relieve the patient of pain, if caries has not spread to the pulp area.

It seems to me that it’s more painful to pull out a tooth (I already managed to face this at 13). Almost 100% sure that your doctor for some reason could not conduct high-quality anesthesia: technical errors, no experience with different types of anesthesia (conduction, infiltration, etc.). If you do not want to suffer further, it is better to change the dentist, since the option of routine reception described by you is not a modern approach to the pulpitis treatment protocol. And “arsenic” is not currently quoted at all, since arsenic paste is harmful to periodontal tooth.

      To summarize: your suffering during treatment is directly related to a violation of the anesthesia technique and the doctor’s inability to use alternative anesthesia options (intraligamentary, intrapulpar, for example, etc.).

      A dentist can even make the anesthesia itself invisible with superficial anesthesia even before the “main” anesthesia is done. Finding a good dentist is not so difficult today. It is enough to learn from your friends their favorite doctors. Believe me, among your relatives and friends there are much more those who trust the doctor and experience much less fears than it was 20-30 years ago. At the appointment with a professional dentist, even those who were previously afraid of injections of anesthesia and pain during treatment, at the end of the procedure are pleasantly surprised that everything went so well. It used to be different: either they did not do anesthesia at all and made them endure, or they used Novocaine, which was like a dead poultice.

    My questions:

    How much more will it hurt for me (I mean the tooth that was removed yesterday)?

    Will it hurt when they treat a tooth? I don’t know if the pain medication will help me, because Yesterday it hurt me terribly - maybe this pain medication didn’t work on me yesterday?

It is possible that you are between 12 and 14 years old, when there is still a chance of having deciduous teeth. If the age is really young, then the dentist needs even more professionalism - I'm talking primarily about the technique of anesthesia. To make qualitative anesthesia, you need not only the help of the patient (adequacy, calmness, etc.), but also the skills of the doctor, as well as a high-quality effective anesthetic (which is not the case in all clinics).
